Please note that it is voluntary for you to provide to the Hong Kong Dental Association (HKDA) your personal data. All personal data submitted will only be used for purposes which are directly related to the processing of your comments, suggestions, enquiries or complaints. In so doing, such personal data may be transferred to the Secretariat of the Community Care Fund, committees under the Community Care Fund, the bureaux/departments of the HKSAR Government or concerned parties involved in the handling of your comments, suggestions, enquiries or complaints. You may state clearly in writing your wish to withhold consent to the disclosure of your personal data to other parties. In this circumstance, we may not be able to process your case satisfactorily or at all.

You have the right to request access to and correction of your personal data held by HKDA. Request for access or correction of personal data should be made in writing to the Project Manager, Office for Community Care Fund Elderly Dental Assistance Programme, HKDA by fax at 2525 8192 or by email to ccfprogram@hkda.org.
